Teacher Reference Form
**Important** The Teacher Reference Forms are required as part of the application process. Please print the Teacher Reference Form above and return 3 reference forms to the Human Resources Department. Family members may not complete reference forms. You must print the entire application and return it in person to the Human Resources Office. Three reference forms are required, and family members may not complete reference forms. The application must be received by 3:30 pm on the Thursday prior to the next scheduled workshop. Once a workshop has reached capacity, the applicant will be able to attend the next scheduled workshop.
